Bones of the Upper Limb: Radius

The radius is longer of the two bones that make up the human antebrachium or forearm. At the proximal end, the radius articulates with the capitulum of the humerus and the radial notch of the ulna to form the elbow joint. At the distal end, the radius articulates with the ulna via the ulnar notch, forming the distal radioulnar joint. Sus antecedentes:

  • Los clavos de la columna medial ofrecen una fijación fuerte para la reconstrucción de la neuroartropatía de Charcot.
  • La colocación percutánea de tornillos puede implicar el riesgo de lesión de los tejidos blandos circundantes.

Objetivo del estudio:

  • Identificar las estructuras anatómicas en riesgo durante la colocación de tornillos de enclavamiento de clavos de la columna medial.
  • Informar la técnica quirúrgica y minimizar la lesión iatrogénica.

Principales métodos:

  • Se insertaron clavos de la columna medial en diez extremidades de cadáver.
  • Se simularon tornillos de enclavamiento con alambres de Kirschner en ubicaciones proximal, media y distal.
  • Se midieron las distancias de los alambres a las estructuras anatómicas y se asignaron niveles de riesgo.

Principales resultados:

  • Proximadamente, el ligamento deltoideo, el tendón tibial posterior y la vena safena estuvieron en riesgo.
  • El nervio cutáneo dorsal medial y la vena marginal medial estuvieron en riesgo a nivel del tornillo medio.
  • El nervio cutáneo dorsal medial estuvo en alto riesgo a nivel del tornillo distal.

Conclusiones:

  • Los tornillos de enclavamiento percutáneos para clavos de la columna medial presentan riesgos significativos para las estructuras neurovasculares y musculotendinosas.
  • Los cirujanos deben ser conscientes de las relaciones anatómicas para mitigar las lesiones durante la reconstrucción del pie de Charcot.
