A continuous quality improvement program for phenytoin i.v

The P & T Committee at Latrobe (Pa.) Area Hospital conducted a drug use review of i.v. phenytoin, which lead to revised guidelines for the use of i.v. loading doses of this agent. The reasons for the study, its results, and the actions taken are presented below.
