Background:

  • Blepharopigmentation, or cosmetic tattooing of the eyelids, is a procedure performed to enhance appearance.
  • Complications, though rare, can occur and may require significant intervention.

Observation:

  • An 81-year-old woman experienced immediate and extensive pigment dispersion from the right lower eyelid lash line to the nasojugal fold after blepharopigmentation.
  • The cosmetic tattoo pigment spread across a wide surface area, rendering nonsurgical treatments ineffective.

Findings:

  • Surgical excision and reconstruction using a lateral canthal suspension and full-thickness skin grafts were required to address the extensive pigment spread.
  • Pathologic examination confirmed pigment infiltration into all skin layers and the orbicularis muscle surface.
