Observation:

  • A 45-year-old male with a history of spinal injury and diabetes presented with fever and scrotal swelling.
  • Ultrasonography revealed epididymitis with abscess formation, necessitating orchiectomy due to suspicion of testicular tumor.
  • Surgical findings showed the epididymis replaced by a yellow mass with abscess, strongly adhered to the testis.

Findings:

  • Histopathological examination confirmed xanthogranulomatous epididymitis, characterized by foamy macrophages and chronic inflammatory cells.
