Area of Science:

  • Geriatric Medicine
  • Endocrinology
  • Internal Medicine

Background:

  • Thyroid dysfunction is common in the elderly.
  • Sick euthyroid syndrome is frequently observed in this population.
  • Assessing thyroid hormone levels on nursing home admission is crucial.

Purpose of the Study:

  • To evaluate the utility of measuring thyroid hormones and TSH in elderly individuals upon admission to a nursing home.
  • To determine the prevalence of thyroid abnormalities in this demographic.

Main Methods:

  • Medical history, physical examination, and geriatric assessment were performed on 201 elderly subjects.
  • Venous blood samples were collected for thyroid hormone and TSH level measurements.
  • Subjects were classified based on cognitive and physical function using Spanish Red Cross Scales.

Main Results:

  • 16 cases (7.9%) of primary hypothyroidism were identified, with 7 (3.5%) requiring L-thyroxine treatment.
  • Sick euthyroid syndrome was diagnosed in 28 cases (13.9%), characterized by normal or low T4 levels.
  • No hyperthyroidism cases were detected, and thyroid abnormalities were not linked to age, gender, or disability status.

Conclusions:

  • Approximately 8% of elderly individuals admitted to nursing homes have hypothyroidism.
  • An additional 14% present with sick euthyroid syndrome.
  • Routine T4 and TSH measurement on admission offers a favorable cost-utility ratio for early detection and management.
