    Background:

    • Meckel's diverticulum is a congenital anomaly that can lead to gastrointestinal bleeding.
    • Diagnosing the source of obscure gastrointestinal hemorrhage can be challenging, especially in elderly patients.

    Observation:

    • A 71-year-old male presented with melena and hemorrhagic shock, indicative of significant gastrointestinal bleeding.
    • Emergent angiography was performed to identify the bleeding source.

    Findings:

    • Angiography revealed contrast media extravasation from the ileal artery.
    • This finding strongly suggested a bleeding Meckel's diverticulum as the cause of the patient's condition.

    Implications:

    • Selective angiography is a valuable tool for diagnosing obscure gastrointestinal bleeding.
