Multiple Lyme disease lesions, erythema migrans, are rare in Europe. This study presents three cases with significantly more lesions than previously reported, highlighting the need for broader differential diagnoses in Europe.

Area of Science:

  • Infectious Diseases
  • Dermatology

Background:

  • Erythema migrans (EM) with multiple lesions is uncommon in European Borrelia infections, typically involving fewer than seven lesions.
  • Previous literature indicates a low incidence (5-20%) of multiple EM lesions in Borrelia infections in Europe.

Observation:

  • Three patients presented with numerous (20-70+) macular, erythematous, centrifugal lesions on the trunk and limbs.
  • Diagnosis of recent Borrelia burgdorferi infection was confirmed via serology, immunoblots, and PCR on skin biopsy.

Findings:

  • The reported cases exhibited a significantly higher number of simultaneous EM lesions than previously documented in European literature.
  • All patients showed an immediate positive response to antibiotic therapy.

Implications:

  • Clinicians in Europe should consider multiple lesion erythema migrans in their differential diagnosis for multiple annular dermatoses.
  • These findings expand the known clinical spectrum of Borrelia burgdorferi infections in Europe.
