Area of Science:

  • Pulmonology
  • Gastroenterology
  • Pharmacology

Background:

  • Ulcerative proctitis is an inflammatory bowel disease.
  • Mesalamine is a common treatment for ulcerative proctitis.
  • Drug-induced lung injury is a potential adverse effect of medications.

Observation:

  • A 23-year-old woman presented with persistent cough, fever, and bilateral lung infiltrates.
  • She had recently been treated with mesalamine for ulcerative proctitis.
  • Antibiotic therapy was ineffective in resolving her lung symptoms.

Findings:

  • Computerized tomography revealed peripheral lung infiltrates.
  • Elevated eosinophil count suggested an allergic or drug-induced etiology.
  • Diagnosis of drug-induced eosinophilic pneumonia was suspected.

Related Experiment Videos

  • Mesalamine withdrawal and corticosteroid treatment led to rapid clinical improvement.
  • Implications:

    • Mesalamine should be considered as a potential cause of eosinophilic pneumonia.
    • Early recognition and drug withdrawal are crucial for managing drug-induced lung injury.
    • This case highlights the importance of a thorough medication history in diagnosing respiratory symptoms.
