Summary

Background:

  • Extensive distal ureter loss can result from radiation fibrosis, operative trauma, or tumor invasion, often complicating treatment for pelvic malignancies.
  • Secondary hydronephrosis and renal compromise are common sequelae, necessitating complex reconstructive strategies.

Observation:

  • Five patients with extensive distal ureteric defects, secondary to pelvic tumor treatment or rectal prolapse, presented with diverse urological complications.
  • Standard ureteral reconstruction methods, including vesical anastomosis and contralateral ureteropyelostomy, were not feasible due to lesion extent, bilateral involvement, or prior bladder treatment.

Findings:

  • Transureteropyelostomy was successfully performed in all five patients as a salvage procedure for distal ureter reconstruction.

  • The postoperative course was largely uneventful, with one transient anastomotic fistula. Four patients achieved long-term survival with excellent clinical and radiological results.

    • Implications:

    • Transureteropyelostomy represents a viable and effective surgical option for managing extensive distal ureteric loss when conventional reconstructive techniques fail.
    • This approach can preserve renal function and improve survival rates in patients with complex ureteral defects following oncological or other pelvic interventions.
