Area of Science:

  • Endocrinology
  • Oncology
  • Pathology

Background:

  • Insular thyroid carcinoma is a rare, poorly differentiated thyroid malignancy.
  • Fewer than 100 cases have been published since its description in 1984.
  • This tumor type is associated with a poor prognosis compared to well-differentiated carcinomas.

Observation:

  • A patient presented with compressive symptoms due to a mediastinal mass.
  • A tumor of the left thyroid lobe, strongly adherent to adjacent structures, was diagnosed as poorly differentiated insular carcinoma.
  • The tumor caused compressive symptoms due to its mediastinal mass.

Findings:

  • The patient underwent total thyroidectomy followed by radioactive iodine (131I) therapy.
  • The patient achieved disease-free status one year after treatment.
  • Diagnosis, treatment, differential diagnosis, and rarity of insular thyroid carcinoma were analyzed.

Implications:

  • Accurate diagnosis is fundamental for aggressive treatment and thorough follow-up.
  • Prompt management is essential due to the aggressive behavior and poor prognosis of insular thyroid carcinoma.
  • This case highlights the importance of timely and aggressive intervention for rare thyroid cancers.
