A 73-year-old woman with chronic cutaneous sarcoidosis achieved complete ulcer healing using Apligraf, a skin equivalent. This innovative treatment avoided the need for systemic steroids in managing her leg ulcers.

Background:

  • Chronic cutaneous sarcoidosis presents diagnostic and therapeutic challenges, particularly when complicated by non-healing ulcers.
  • Widespread skin lesions over 25 years without systemic involvement are noted.
  • Ulceration of sarcoid lesions on the legs, refractory to topical treatments, necessitates advanced therapeutic strategies.

Observation:

  • A 73-year-old female patient with a 25-year history of cutaneous sarcoidosis developed ulcerated lesions on her legs.
  • Standard investigations ruled out venous insufficiency or other common causes for the leg ulcers.
  • Histopathology confirmed non-caseating granulomas, characteristic of sarcoidosis, in both plaque and ulcer surrounding tissues.

Findings:

  • Despite leukopenia and elevated serum angiotensin-converting enzyme, systemic sarcoidosis was not evident.
