Summary

A rare CD8-positive cutaneous lymphoma mimicked pyoderma gangrenosum in a patient. Radiotherapy led to complete remission, with no relapse after 28 months, highlighting an extraordinary clinical presentation and successful treatment.

Background:

  • Primary cutaneous lymphomas (PCLs) are a heterogeneous group of non-Hodgkin lymphomas.
  • Ulcerated PCLs are known, but presentation mimicking pyoderma gangrenosum is exceptionally rare.
  • CD8-positive lymphomas represent a rare subtype with variable clinical outcomes.

Observation:

  • A 49-year-old patient presented with a large, ulcerated lesion on the chest wall.
  • The lesion clinically resembled pyoderma gangrenosum.
  • Immunohistochemical staining revealed a CD8-positive lymphocyte infiltrate, with absent CD30 expression.

Findings:

  • The patient was diagnosed with an unclassifiable, ulcerated, CD8-positive primary cutaneous lymphoma.
  • Treatment with radiotherapy, specifically complete skin irradiation, resulted in complete regression of the lymphoma.
