Summary

A mediastinal tumor, diagnosed as a bronchogenic cyst, was successfully removed. Postoperative chylothorax was managed with a low-fat diet, resolving within 10 days.

Observation:

  • A 27-year-old male presented with an abnormal chest X-ray finding.
  • Chest computed tomography (CT) revealed a localized mediastinal tumor.
  • The tumor was surgically resected via thoracoscopy and pathologically identified as a bronchogenic cyst.

Findings:

  • Postoperative chylothorax developed on day 1 after tumor resection.
  • Conservative management with a low-fat diet was initiated for chyliform pleural effusion.
  • Pleural effusion significantly reduced, allowing for drainage tube removal on postoperative day 10.

Implications:

  • This case highlights successful thoracoscopic resection of a mediastinal bronchogenic cyst.
  • Effective management of postoperative chylothorax with dietary modification is demonstrated.
  • Long-term follow-up confirmed no recurrence, indicating favorable surgical outcomes for mediastinal bronchogenic cysts.
