Summary

Epstein-Barr virus (EBV) rarely replicates in tongue epithelial cells in non-AIDS patients. Immune suppression or severe illness may enable EBV replication in these cells.

Background:

  • Epstein-Barr virus (EBV) establishes lifelong B-cell infections and can be present in saliva.
  • The specific cells responsible for infectious EBV shedding remain unclear.
  • EBV replication in oral epithelial cells, as seen in hairy leukoplakia, has been observed but its general significance is uncertain.

Purpose of the Study:

  • To investigate the prevalence and cellular sites of Epstein-Barr virus (EBV) replication in the oropharynx, specifically tongue epithelial cells.
  • To determine if tongue squamous epithelial cells can support EBV replication in individuals without AIDS.

Main Methods:

  • Immunohistochemistry and in situ hybridization techniques were employed.
  • Analysis was conducted on 168 tongue tissue samples from 84 autopsy cases.

    • Evidence of EBV replication in tongue epithelial cells was detected in 4 out of 168 samples (approximately 2.4%).

    • This suggests that impaired immune function may be a prerequisite for EBV replication in this specific epithelial site.

    Conclusions:

    • Squamous epithelial cells of the tongue infrequently support Epstein-Barr virus (EBV) replication in the general population.
    • Immune system compromise appears to be a critical factor enabling EBV replication in oropharyngeal epithelial cells.
    • These findings clarify the limited role of tongue epithelium in EBV replication under normal immune conditions.
