Multiple sclerosis (MS) rarely causes movement disorders. This study details a rare case of a simple phonic tic, like throat-clearing, in a patient with MS, linked to brain lesions.

Area of Science:

  • Neurology
  • Neuroimmunology
  • Movement Disorders

Background:

  • Multiple sclerosis (MS) is a chronic inflammatory demyelinating disease of the central nervous system.
  • Movement disorders are uncommon manifestations of MS, with paroxysmal dystonia being the most frequent.
  • Various other movement abnormalities, including chorea and dystonia, have been anecdotally reported in MS patients.

Observation:

  • A 34-year-old woman with a confirmed diagnosis of multiple sclerosis presented with a new onset of a simple phonic tic.
  • The patient's tic was characterized by repetitive throat-clearing sounds.
  • Neurological examination and magnetic resonance imaging (MRI) were performed to investigate the cause.

Findings:

  • MRI revealed demyelinating lesions specifically located in the thalamus and basal ganglia.

Related Experiment Videos

  • These brain regions are critical for motor control and sensory processing.
  • The clinical presentation of a phonic tic was directly correlated with the observed lesions.
  • Implications:

    • This case represents the first documented instance of a tic disorder as a direct clinical manifestation of multiple sclerosis.
    • It expands the spectrum of known neurological symptoms associated with MS.
    • Highlights the importance of considering MS in the differential diagnosis of movement disorders, particularly in younger adults with relevant lesions.
