Nephrotoxic effects in high-risk patients undergoing angiography

Iodixanol, an iso-osmolar contrast medium, significantly reduced contrast-induced nephropathy in high-risk diabetic patients compared to iohexol, a low-osmolar agent. This suggests iodixanol may be a safer choice for preventing kidney damage in vulnerable populations.

Purpose of the Study:
- To compare the nephrotoxic effects of iodixanol (iso-osmolar) and iohexol (low-osmolar) in high-risk patients undergoing angiography.
- To evaluate the safety of different osmolar contrast media in preventing kidney injury.
Main Methods:
- A randomized, double-blind, prospective, multicenter study.
- 129 diabetic patients with elevated serum creatinine were randomized to receive either iodixanol or iohexol.
- Primary endpoint: peak increase in serum creatinine within 3 days post-angiography.
Main Results:
- Iodixanol group showed significantly less increase in creatinine levels compared to the iohexol group (mean peak increase: 0.13 vs 0.55 mg/dL, P=0.001).
- Fewer patients in the iodixanol group experienced a ≥0.5 mg/dL creatinine increase (3% vs 26%, P=0.002).
- No patient receiving iodixanol had a ≥1.0 mg/dL increase, versus 15% in the iohexol group.
Conclusions:
- Iodixanol may be less nephrotoxic than low-osmolar contrast media in high-risk patients.
- The use of iodixanol could reduce the incidence of contrast-induced nephropathy in vulnerable patient populations.
