Summary

Endometriosis is diagnosed in 26% of infertile women, but dysmenorrhea (painful periods) is not a reliable indicator for diagnosing endometriosis. Pain symptoms did not correlate with endometriosis stage or location.

Area of Science:

  • Gynecology and Reproductive Medicine
  • Surgical Pathology
  • Pain Management

Context:

  • Endometriosis, a condition affecting women of reproductive age, is characterized by pelvic pain, adnexal tumors, and infertility.
  • Key symptoms include dysmenorrhea, dyspareunia, and pain during defecation, often linked to peritoneal inflammation and fibrotic reactions.
  • Laparoscopy is a primary diagnostic tool for endometriosis, enabling direct visualization and staging of the disease.

Purpose:

  • To investigate the prevalence of endometriosis in infertile women compared to fertile controls.
  • To evaluate the correlation between painful menstruation (dysmenorrhea) and the presence, stage, and location of endometriosis.
  • To assess the diagnostic accuracy of dysmenorrhea as a symptom for identifying endometriosis.

Summary:

  • Laparoscopy diagnosed endometriosis in 26% of infertile women versus 5% of fertile women (p < 0.001).
  • Dysmenorrhea prevalence was similar in infertile women with (46.92%) and without endometriosis (48.68%), indicating low diagnostic value.
  • No significant correlation was found between dysmenorrhea and endometriosis stage or implant location, with sensitivity at 47% and specificity at 51%.

Impact:

  • This study highlights the limitations of relying solely on dysmenorrhea for endometriosis diagnosis in infertile populations.
  • Findings underscore the necessity of diagnostic laparoscopy for accurate endometriosis assessment.
  • Results contribute to a better understanding of symptom-disease correlation, aiding in clinical decision-making and patient counseling.
