Summary

This study evaluated frontolateral laryngectomy for T1a-b glottic carcinomas with commissure extension. Surgical variations improved functional and oncological outcomes in 21 patients over a 7-year follow-up.

Area of Science:

  • Otolaryngology
  • Surgical Oncology

Background:

  • Glottic carcinomas (T1a-b) with commissure extension require specialized surgical approaches.
  • Frontolateral laryngectomy is a recognized technique for managing these tumors.

Observation:

  • A series of 21 patients with T1a-b glottic carcinomas and commissure extension were treated between 1991 and 2000.
  • Eight patients underwent standard Leroux-Robert frontolateral laryngectomy.
  • Thirteen patients received modified frontolateral laryngectomy incorporating superficial cervical fascia and vestibular band reconstruction.

Findings:

  • The study assessed functional and oncological results following surgical intervention.
  • Follow-up ranged from a minimum of one year to a maximum of seven years.
  • Modified techniques demonstrated promising functional and oncological outcomes (specific results pending full analysis).

Implications:

  • Surgical modifications to frontolateral laryngectomy may enhance outcomes for glottic carcinoma.
  • These findings could inform surgical planning for similar cases.
  • Further research is warranted to validate the long-term efficacy of these reconstructive techniques.
