Acanthamoeba keratitis in contact lens wearers is challenging to treat. While two of three patients recovered with a combination of topical and oral medications, one experienced persistent corneal issues.

Area of Science:

  • Ophthalmology
  • Infectious Diseases

Background:

  • Acanthamoeba keratitis (AK) is a severe microbial infection of the cornea.
  • Contact lens wear is a significant risk factor for AK development.

Observation:

  • Three cases of AK were observed in contact lens wearers within a year.
  • Diagnosis was confirmed through clinical presentation and laboratory cultures of corneal and conjunctival swabs.

Findings:

  • Patients received a multifaceted treatment regimen including topical medications (metronidazole, propamidine isethionate, neomycin, corticosteroids, mydriatics, beta-blockers) and oral itraconazole.
  • Two patients achieved resolution of the infection.
  • One patient continued to experience recurrent corneal erosions and significant corneal immune response.

Implications:

  • Early diagnosis and aggressive treatment are crucial for managing Acanthamoeba keratitis.
  • Despite treatment, some patients may face long-term complications, highlighting the need for further research into optimal therapeutic strategies.
