Summary

Severe chronic actinic dermatitis can trigger vitiligo-like depigmentation. This condition involves the destruction of melanocytes by cytotoxic T-cells, leading to pigment loss.

Background:

  • Chronic actinic dermatitis is a severe skin condition.
  • Vitiligo-like depigmentation is an unusual complication.

Observation:

  • Two patients with severe chronic actinic dermatitis developed widespread vitiligo-like depigmentation.
  • Lesion progression was linked to the underlying dermatitis.
  • Depigmented skin showed a complete absence of tyrosinase-positive melanocytes.

Findings:

  • Immunohistology revealed a predominance of CD-8 positive cytotoxic/-suppressor lymphocytes in the inflammatory infiltrate.
  • Dense exocytosis of CD-8 T-cells was observed in the epidermis adjacent to depigmenting lesions.
  • This suggests a cytotoxic mechanism targeting melanocytes.
