Melancholic depression is linked to reduced subgenual prefrontal cortex (PFC) activity, indicated by altered brainwaves and metabolism. This PFC dysfunction may explain anhedonia and stress responses in melancholia.

  • Major depression is heterogeneous, with inconclusive neural correlates for subtypes.
  • Subgenual prefrontal cortex (PFC) dysfunction is theoretically linked to melancholia, a subtype characterized by anhedonia.
  • Empirical evidence for this link has been lacking.

Purpose of the Study:

  • To test the hypothesis that melancholic depression, not nonmelancholic depression, is associated with subgenual PFC impairment.
  • To investigate the neural underpinnings of melancholia using concurrent electroencephalogram (EEG) and positron emission tomography (PET).

Main Methods:

  • Concurrent EEG and PET measurements in unmedicated depressed subjects (melancholic and nonmelancholic) and controls.
  • EEG analysis using tomographic source localization to assess cortical current density.

    • Antidepressant treatment response correlated with reduced subgenual PFC delta activity.
    • No structural differences in the subgenual PFC were found between groups, but melancholic subjects showed an age-related decrease in gray matter density.

    Conclusions:

    • Subgenual PFC dysfunction is a neural correlate of melancholic depression.
    • This dysfunction may contribute to anhedonia and heightened stress responsiveness in melancholia.
    • Subgenual PFC activity, particularly delta wave patterns, may serve as a biomarker for treatment response.
