    Background:

    • Strongyloidiasis is caused by the parasitic nematode Strongyloides stercoralis.
    • Traditionally considered rare in non-endemic regions like Moscow.

    Observation:

    • Two cases of Strongyloides stercoralis infection were identified in the Moscow Region.
    • The patients were a 13-year-old boy and a 93-year-old female.

    Findings:

    • The cases suggest a potential resurgence of strongyloidiasis in the Moscow Region.
    • Moscow strains appear to have low virulence but high autoinvasion capacity, leading to chronic infections.
    • Diagnostic challenges exist due to low invasion rates and inefficient parasitological methods.

    Implications:

    • Strongyloidiasis should be considered in patients with unexplained allergic reactions, eosinophilia, or gastrointestinal symptoms, regardless of travel history.
    • A syndromic-and-empiric approach to chemotherapy is recommended due to diagnostic limitations.
    • Increased awareness and diagnostic vigilance are necessary for this re-emerging infection.
