Area of Science:

  • Cardiovascular Surgery
  • Thoracic Surgery
  • Vascular Surgery

Context:

  • Aortic arch pathologies require complex surgical interventions.
  • Median sternotomy is a common surgical approach for thoracic procedures.

Purpose:

  • To evaluate the safety and efficacy of total aortic arch replacement via a median sternotomy approach.
  • To assess the operative risk associated with this surgical technique.

Summary:

  • Seven patients underwent total aortic arch replacement using a median approach with Dacron grafts.
  • Distal anastomosis was achieved via open technique with lower body circulatory arrest or balloon occlusion.
  • Arch vessel anastomosis was performed after coronary reperfusion, with no early operative deaths.

Impact:

  • Demonstrates the feasibility of total aortic arch replacement through a median approach.
  • Suggests a low operative risk profile for this surgical strategy.
  • Provides evidence for surgeons considering median sternotomy for aortic arch reconstruction.
