Summary
Histopathologic classification of primary cutaneous germinal center cell lymphomas (PCGCCL) shows poor reproducibility due to cell pleomorphism. Morphometric analysis revealed overlapping cell parameters, suggesting current classification systems need revision for these B-cell neoplasias.
Area of Science:
- Hematology
- Oncology
- Dermatopathology
Background:
- Primary cutaneous germinal center cell lymphomas (PCGCCL) are B-cell neoplasms with significant cellular pleomorphism.
- Histopathologic examination of mixed-cell-type PCGCCL exhibits poor interobserver reproducibility.
- The heterogeneity in cell size and nuclear morphology complicates accurate classification.
Purpose of the Study:
- To evaluate if morphometric analysis can substantiate light microscopic criteria for classifying cleaved and noncleaved cells in PCGCCL.
- To assess the reproducibility of morphometric analysis in differentiating cell subsets within these lymphomas.
- To determine the suitability of current classification systems for PCGCCL.
Main Methods:
- Utilized an image analyzer (IBAS 2000) for morphometric analysis.
- Studied 17 cases of centroblastic/centrocytic and centroblastic follicular and/or diffuse primary cutaneous lymphomas.
- Quantified nuclear parameters of neoplastic cells to differentiate subtypes.
Main Results:
- Morphometric analysis revealed distinct differences between follicular and diffuse patterns of neoplastic cells.
- Follicular neoplasms showed smaller, more cleaved, and monomorphic cells compared to diffuse forms.
- A unimodal cell population with intermediate morphometric features was observed, indicating significant overlap.
Conclusions:
- Considerable overlap in nuclear parameters between cell subtypes in cutaneous mixed lymphomas exists.
- The Kiel classification and Working Formulation are unsuitable for PCGCCL.
- Revision of classification criteria for cutaneous germinal center cell lymphomas is supported by these findings.
