Horizontal supraglottic laryngectomy (HSL) offers excellent oncological outcomes for supraglottic squamous cell carcinoma. While functional recovery is generally good, careful patient selection is crucial due to potential postoperative morbidity.

Area of Science:

  • Otolaryngology
  • Head and Neck Surgery
  • Oncology

Background:

  • Supraglottic squamous cell carcinoma (SCC) requires effective treatment balancing oncological control and laryngeal function.
  • Horizontal supraglottic laryngectomy (HSL) is a surgical option for this condition.

Purpose of the Study:

  • To evaluate the oncological outcomes and functional results of horizontal supraglottic laryngectomy (HSL) for supraglottic SCC.
  • To assess factors influencing survival and recurrence.
  • To compare HSL with endoscopic laser surgery.

Main Methods:

  • Retrospective study of 75 patients with supraglottic SCC treated with HSL between 1981-1999.
  • Analysis of tumor stage, neck disease, surgical procedures (neck dissection, adjuvant radiotherapy), and follow-up data.

Related Experiment Videos

  • Evaluation of disease-specific survival, locoregional control, and functional outcomes (aspiration, diet, decannulation).
  • Main Results:

    • Five-year disease-specific survival was 92%, with high locoregional (90%) and local (92.5%) control rates.
    • Cartilage infiltration was associated with local control and survival.
    • Postoperative aspiration occurred in 26% of patients, with most recovering function; no permanent tracheostomy was required.

    Conclusions:

    • Horizontal supraglottic laryngectomy (HSL) provides excellent oncological results for supraglottic SCC.
    • Functional morbidity is notable, underscoring the importance of patient selection.
    • Endoscopic laser surgery may offer comparable outcomes with less morbidity and warrants prospective comparison.
