Epithelioid sarcoma in the chest wall is rare. Even small primary tumors can metastasize within the same muscle, requiring careful consideration during diagnosis and treatment.

Area of Science:

  • Oncology
  • Surgical Pathology

Background:

  • Epithelioid sarcoma is a rare soft tissue sarcoma with a predilection for extremities.
  • Chest wall involvement by epithelioid sarcoma is exceptionally uncommon.

Observation:

  • Case 1: A 64-year-old male presented with a large epithelioid sarcoma in the left erector spinae muscle, experiencing multiple recurrences and eventual systemic metastasis.
  • Case 2: A 24-year-old female had a smaller epithelioid sarcoma in the left seventh intercostal muscle with a subsequent metastasis within the same muscle group.

Findings:

  • The first case demonstrated aggressive local recurrence and distant spread despite wide surgical resections.
  • The second case, despite initial metastasis to the ipsilateral intercostal muscles, showed no recurrence after resection of adjacent intercostal muscles.

Related Experiment Videos

Implications:

  • Chest wall epithelioid sarcoma can exhibit aggressive behavior and a propensity for local recurrence and metastasis.
  • Metastasis within the primary muscle of origin should be suspected in chest wall epithelioid sarcoma, irrespective of primary tumor size.
  • Aggressive surgical management, potentially involving wider resection margins, may be necessary for optimal outcomes.
