Summary

The Stretta procedure offers a safe, endoscopic treatment for GERD, modestly improving symptoms and reducing acid exposure in 60% of patients. Long-term follow-up shows sustained benefits for responders.

Area of Science:

  • Gastroenterology
  • Minimally Invasive Surgery
  • Gastroesophageal Reflux Disease (GERD) Management

Background:

  • Endoscopic treatments are emerging for GERD, but long-term data is limited.
  • The Stretta procedure utilizes endoscopic radiofrequency energy for GERD treatment.
  • This study presents a 3-year experience with the Stretta procedure at Vanderbilt University.

Purpose of the Study:

  • To evaluate the long-term efficacy and safety of the Stretta procedure for GERD.
  • To assess patient outcomes, including symptom improvement, medication use, and satisfaction.
  • To correlate objective measures of acid reflux with subjective patient responses.

Main Methods:

  • Prospective study of 85 patients undergoing the Stretta procedure with >6 months follow-up.

Related Experiment Videos

  • Patients completed quality-of-life (QOLRAD), SF-12 health status, and satisfaction questionnaires.
  • 24-hour pH studies were performed pre- and post-procedure to measure acid exposure.
  • Main Results:

    • 60% of patients were responders (off or >50% PPI dose reduction) at 26.2 +/- 7.5 months follow-up.
    • Responders showed significant improvement in acid exposure (p=0.0001) and DeMeester scores (p=0.016).
    • Patient satisfaction was 73%, with responders reporting significantly better SF-12 scores than non-responders.

    Conclusions:

    • The Stretta procedure is a safe and modestly effective endoscopic treatment for GERD.
    • Symptomatic improvement correlates with reduced distal acid exposure, normalizing in nearly half of patients.
    • Long-term follow-up indicates sustained benefits for a significant portion of treated GERD patients.
