Summary
Corticosteroids did not improve outcomes for acute traumatic brain injury and may increase mortality risk. These findings suggest routine steroid use in traumatic head injury patients should be discontinued.
Area of Science:
- Neuroscience
- Clinical Medicine
- Pharmacology
Background:
- Traumatic brain injury (TBI) is a significant cause of mortality and long-term disability.
- Corticosteroids have been historically employed in the management of acute TBI.
Purpose of the Study:
- To systematically evaluate the efficacy and safety of corticosteroid administration in patients with acute traumatic brain injury.
- To synthesize evidence from randomized controlled trials regarding corticosteroid treatment for TBI.
Main Methods:
- A comprehensive literature search was conducted across multiple electronic databases (MEDLINE, EMBASE, Cochrane Library) up to October 2004.
- Included were randomized controlled trials (RCTs) assessing corticosteroid use in acute TBI with adequate or unclear allocation concealment.
- Data extraction focused on participant numbers, follow-up duration, mortality, disability, infection rates, and gastrointestinal bleeding.
Main Results:
- Twenty RCTs involving 12,303 participants were identified.
- The largest trial, comprising 80% of participants, indicated a significant increase in mortality risk with steroid use (Risk Ratio: 1.18; 95% CI: 1.09–1.27).
- Pooled analysis for death or severe disability showed a relative risk of 1.01 (0.91–1.11), while infection risk was 1.03 (0.99–1.07), and gastrointestinal bleeding risk was 1.23 (0.91–1.67).
Conclusions:
- The substantial increase in mortality observed in the largest trial warrants careful consideration.
- Current evidence suggests that corticosteroids should not be routinely administered to patients with acute traumatic brain injury.
- Discontinuation of routine corticosteroid use in TBI management is recommended based on safety and efficacy data.
