Antibiotics for acute pyelonephritis in children
Children with acute pyelonephritis (a severe urinary tract infection) can be effectively treated with oral cefixime or short courses of intravenous (IV) therapy. Daily IV aminoglycoside dosing is also safe and effective for acute pyelonephritis treatment.
Area of Science:
- Pediatric Nephrology
- Infectious Diseases
- Clinical Pharmacology
Background:
- Urinary tract infections (UTIs) are common in infants, with acute pyelonephritis being the most severe form.
- Acute pyelonephritis can lead to significant morbidity and permanent kidney damage.
- Current guidelines recommend initial intravenous (IV) followed by oral antibiotic therapy, but optimal duration remains unclear.
Purpose of the Study:
- To evaluate the benefits and harms of various antibiotic regimens for treating acute pyelonephritis in children.
- To compare different antibiotic agents, routes, frequencies, and durations of therapy.
Main Methods:
- Systematic review of randomized and quasi-randomized controlled trials.
- Searched major databases (Cochrane, MEDLINE, EMBASE) without language restrictions.
- Data extraction and quality assessment by two independent reviewers; random-effects model used for analysis.
Main Results:
- No significant difference in renal damage or fever duration between 14-day oral cefixime and 3-day IV followed by 10-day oral therapy.
- No significant difference in renal damage between 3-4 day IV followed by oral therapy and 7-14 day IV therapy.
- Efficacy of daily versus thrice-daily aminoglycoside administration showed no significant difference.
Conclusions:
- Children with acute pyelonephritis can be effectively treated with oral cefixime or short-course IV therapy (2-4 days) followed by oral antibiotics.
- Single daily dosing of aminoglycosides is a safe and effective IV option.
- Further trials are needed to determine optimal total therapy duration and the role of other oral antibiotics in initial treatment.
