Summary

Routine postoperative chest X-rays (CXRs) are not necessary after open nephrectomy. Selective CXRs are recommended only when specific clinical indications arise, improving patient safety and reducing radiation exposure.

Background:

  • Routine postoperative chest radiography (CXR) is a common practice after open nephrectomy to detect pneumothorax.
  • However, the clinical utility and necessity of routine CXRs in this context are questionable, with low incidence of pneumothorax and minimal impact on management at some institutions.

Purpose of the Study:

  • To evaluate the requirement of routine postoperative chest radiography (CXR) following open nephrectomy.
  • To determine if routine CXRs are effective in detecting and managing postoperative pneumothorax.

Main Methods:

  • A retrospective analysis of 150 open nephrectomy cases performed by two urologists was conducted.
  • Laparoscopic and thoracoabdominal nephrectomies were excluded from the study.

  • Out of 150 patients, 92 underwent postoperative CXR, revealing 4 cases (2.7%) of pneumothorax.
  • Pneumothorax occurred in patients with recognized (3 cases) or unrecognized (1 case) pleural tears during surgery.
  • Only one patient required a chest tube for pneumothorax management.

Conclusions:

  • Routine postoperative CXRs are not indicated after open nephrectomy.
  • Selective CXR is a safe, cost-effective, and radiation-sparing approach when specific clinical criteria are met.
