Summary

Immunohistochemistry aids in distinguishing between epidermoid and mesothelial splenic cysts (ESC). This study suggests these cysts may have different origins, challenging existing theories and highlighting new diagnostic and treatment approaches for these rare spleen lesions.

Background:

Epithelial splenic cysts (ESC) are rare splenic lesions with debated origins.
Distinguishing between epidermoid and mesothelial cysts can be challenging histologically.

Purpose of the Study:

To evaluate the utility of immunohistochemistry in differentiating epidermoid and mesothelial splenic cysts.
To explore the etiopathogenesis of epithelial splenic cysts.

Main Methods:

Retrospective review of 11 patients with epithelial splenic cysts (1986-2004).
Detailed histological analysis with immunohistochemistry for CEA, CA 19-9, cytokeratin, and calretinin.

Main Results:

Epidermoid cysts showed positivity for CEA, CA 19-9, and cytokeratin; negative for calretinin.

Mesothelial cysts were positive for cytokeratin and calretinin; negative for CEA and CA 19-9.

Conclusions:

Immunohistochemistry provides a reliable method for differential diagnosis of epidermoid and mesothelial cysts.
Findings suggest distinct origins for epidermoid and mesothelial cysts, potentially challenging Burrig's theory.
While traditional open splenectomy was used, conservative and laparoscopic techniques offer promising alternatives for ESC management.
