Background:

  • Spinal surgery trends towards reduction and minimalization, but traditional methods can have sequelae.
  • Plasma-mediated electrosurgery is a well-suited technology for minimally invasive spinal interventions.
  • Percutaneous disc decompression (PDD) and Coblation-Assisted Microdiscectomy (CAM) utilize this technology.

Purpose of the Study:

  • To evaluate the clinical outcomes of Nucleoplasty (Coblation) and CAM for treating contained disc herniation.
  • To assess the efficacy of these minimally invasive techniques in reducing pain and improving function.
  • To compare the results of PDD using coblation technology with CAM procedures.

Main Methods:

  • A small sample of 64 patients with contained disc herniation underwent either PDD (Nucleoplasty) or CAM.
  • Procedures were performed on an outpatient basis using the Perc-DLE SpineWand and System 2000 generator.
  • Follow-up data was collected over 1 to 12 months to assess pain scores and clinical improvement.

Main Results:

  • Nucleoplasty (PDD) showed significant improvement, with 80% of patients experiencing reduced pain scores at 6-12 months.
  • 75% of PDD patients reported very good improvement, 5% good, and 15% showed some improvement.
  • No patients in the PDD group reported worsening of their condition; CAM was effective for root compression and spine stenosis.

Conclusions:

  • Nucleoplasty (Coblation) appears to be an efficacious minimally invasive technique for symptomatic contained herniated discs.
  • CAM is an excellent method for addressing root compression and spine stenosis.
  • Randomized controlled studies are necessary to precisely define the role of Nucleoplasty in spinal treatment.
