    Background:

    • Memantine, a glutamate NMDA receptor antagonist, is investigated for its potential to mitigate excitotoxic neurotoxicity in dementia.
    • Dementia, including Alzheimer's disease (AD) and vascular dementia (VD), poses significant challenges in cognitive and functional decline.

    Purpose of the Study:

    • To evaluate the efficacy and safety of memantine in patients with Alzheimer's disease (AD), vascular dementia (VD), and mixed dementia.
    • To synthesize evidence from randomized controlled trials on memantine's impact on cognitive function, daily living activities, and behavior.

    Main Methods:

    • A systematic search of the Cochrane Dementia and Cognitive Improvement Group Specialized Register and other databases was conducted.
    • Double-blind, placebo-controlled, randomized trials of memantine in individuals with dementia were included.
    • Data from eligible studies were pooled for intention-to-treat (ITT) and observed case (OC) analyses.

    Main Results:

    • In moderate to severe AD, published data indicate a small beneficial effect of memantine on cognition, daily living, and behavior at six months.
    • For mild to moderate AD, memantine demonstrated a beneficial effect on cognition and behavior in ITT analysis, but results were inconclusive due to unpublished data.
    • In mild to moderate vascular dementia, memantine improved cognition and behavior, though not consistently supported by global clinical measures. Patients on memantine showed reduced incidence of agitation.

    Conclusions:

    • Published evidence suggests a modest short-term benefit of memantine in moderate to severe Alzheimer's disease.
    • The clinical significance of memantine's cognitive benefits in vascular dementia remains uncertain.
    • The efficacy of memantine in mild to moderate Alzheimer's disease is currently unknown due to conflicting data from published and unpublished studies.
