Background:

  • The Rh blood group system is complex, with numerous antigens and phenotypes.
  • Rare Rh phenotypes, such as Rhnull and deletion types, are clinically significant.

Purpose of the Study:

  • To characterize a novel red blood cell (RBC) phenotype, Rh:-32,-46.
  • To investigate the serological properties of the alloantibody found in the donor's serum.
  • To compare this finding with previously described Rh:-46 RBCs.

Main Methods:

  • Standard blood group typing techniques were employed.
  • Serological testing included reactivity with various RBCs and enzyme/chemical treatments (ficin, papain, AET, DTT).

    • A white male donor presented with a rare Rh:-32,-46 RBC phenotype, not associated with known Rh deletion or Rhnull types.

    • Reactivity was enhanced by ficin/papain treatment and unaffected by AET/DTT.

    Conclusions:

    • This is the first report of an Rh:-32,-46 phenotype not linked to rare Rh deletion or Rhnull types.
    • The findings expand the understanding of Rh antigen diversity and alloantibody formation.
    • The rare Rh:w5,w19,-31,-34 phenotype observed in the donor is also noteworthy in Caucasian populations.
