A rare case of subcutaneous alternariosis caused by Alternaria alternata occurred in a kidney transplant recipient. The fungal infection presented as a large forearm mass, confirmed by biopsy and culture.

Area of Science:

  • Mycology
  • Dermatology
  • Transplant Infectious Diseases

Background:

  • Subcutaneous fungal infections can pose a significant challenge in immunocompromised individuals.
  • Kidney transplant recipients are at increased risk for opportunistic infections due to immunosuppressive therapy.

Observation:

  • A 52-year-old male kidney transplant recipient developed a 7 cm vegetating mass on his right forearm.
  • Histopathology revealed fungal elements consistent with Alternaria alternata and chronic granulomatous inflammation.

Findings:

  • The causative agent, Alternaria alternata, was identified through macroscopic and microscopic characterization of fungal colonies grown from biopsy.
  • Grocott staining and periodic acid-Schiff diastase confirmed the presence of fungal spores and hyphae.

Related Experiment Videos

Implications:

  • This case highlights Alternaria alternata as a potential pathogen in subcutaneous infections post-kidney transplantation.
  • Early diagnosis and appropriate antifungal treatment are crucial for managing such infections in immunocompromised patients.
