Summary

BB gun injuries can cause both optic nerve transection and chorioretinitis sclopetaria, a rare combination. This case highlights the severe ocular trauma possible from such incidents.

Background:

  • Blunt ocular trauma can lead to optic nerve transection or chorioretinitis sclopetaria.
  • Simultaneous occurrence of these conditions is rare and previously unreported.

Observation:

  • A 12-year-old boy presented with vision loss after a BB gun injury to the right eye.
  • Examination revealed a pale optic disc, retinal scarring, submacular fibrosis (chorioretinitis sclopetaria), and a relative afferent pupillary defect.
  • Orbital CT confirmed optic nerve transection by the BB pellet lodged at the orbital apex.

Findings:

  • The case represents the first reported instance of concomitant optic nerve transection and chorioretinitis sclopetaria.
  • BB gun projectiles can cause severe, combined ocular and orbital trauma.
