Routine bowel preparations before intravenous pyelogram (IVP) do not improve image quality. Laxative preparations may offer slight benefits for visualizing the right kidney but cause side effects.
Area of Science:
- Radiology
- Gastroenterology
- Clinical Trials
Background:
- Intravenous pyelogram (IVP) is a common diagnostic imaging procedure.
- Bowel preparation is often performed before IVP to enhance visualization.
- The necessity and optimal method of bowel preparation for IVP remain debated.
Purpose of the Study:
- To evaluate the necessity of routine bowel preparations for satisfactory urinary system visualization during IVP.
- To compare the side effects of different bowel preparation methods.
Main Methods:
- A prospective, randomized, single-blind, controlled trial involving 200 patients undergoing IVP.
- Four groups received different bowel preparations: liquid supper, senna, magnesium sulfate, or PEG-ELS.
- Radiologists and urologists, blinded to preparation, assessed image quality (visualization, fecal residue, bowel gas, overall quality).
Main Results:
- No significant differences in image quality (contrast or control) were observed among the four groups.
- Senna and PEG-ELS groups showed lower fecal residue scores than the liquid supper group on control images.
- Laxative preparations (senna, magnesium sulfate, PEG-ELS) resulted in reported side effects, with no preference for re-ingestion among these groups.
Conclusions:
- Routine bowel preparation before IVP does not enhance overall image quality.
- Laxative bowel preparations may offer limited benefit for right renal region visualization on control images.
- Bowel preparations are associated with patient side effects, questioning the routine necessity.
