Summary

This study reports the first case of cerium-induced granulomatous dermatitis, a rare skin reaction to a common burn treatment. Hydroxychloroquine showed positive results in managing this cerium side effect.

Background:

  • Inorganic foreign bodies are occasionally found in granulomatous skin lesions.
  • Cutaneous granulomatous dermatitis is a recognized condition, but cerium-induced cases were previously undocumented.

Observation:

  • A 57-year-old woman developed papulonodular lesions in burn scars treated with cerium nitrate-silver sulfadiazine cream.
  • Biopsies confirmed sarcoidal granuloma with exogenous particles, and microanalysis revealed high cerium content.

Findings:

  • The patient presented with cerium-induced cutaneous granulomatous dermatitis, a novel finding.
  • Systemic sarcoidosis was ruled out; the condition was localized to treated burn areas.
  • Treatment with hydroxychloroquine led to clinical and histological improvement.

  • Individual immunologic status may influence the development of granulomatous reactions to foreign substances.
