Transbronchial biopsy in usual interstitial pneumonia

Background:

• Usual interstitial pneumonia (UIP) is a progressive, fatal idiopathic interstitial pneumonia with no effective treatment.
• Transbronchial biopsy (TBB) has historically been used to exclude other conditions, not confirm UIP.

Purpose of the Study:

• To evaluate if diagnostic features of UIP are discernible in TBB specimens.
• To assess the diagnostic utility of TBB for UIP.

Main Methods:

• Retrospective analysis of TBB specimens from patients with confirmed UIP.
• Study included 22 patients with UIP confirmed by surgical lung biopsy, lung explant, or clinical/radiographic findings.

Main Results:

• Diagnostic UIP features (patchy interstitial fibrosis, fibroblast foci, honeycomb change) were identified in 32% of adequate TBB specimens.
• An additional 9% of cases showed features consistent with UIP.
• Nonspecific interstitial fibrosis was found in the remaining cases.

Conclusions:

• Characteristic histologic features of UIP can be identified on TBB specimens.
• TBB may play a more significant role in confirming UIP diagnoses than previously recognized.
