Summary

Electroencephalograms (EEGs) reveal that epileptiform activity in Rett syndrome is highest during light non-rapid eye movement sleep and early morning hours. These sleep EEG patterns may aid in diagnosing this neurological disorder.

Background:

  • Rett syndrome is a rare neurodevelopmental disorder primarily affecting girls.
  • Epileptiform activity is a common feature in Rett syndrome, but its relationship with sleep architecture is not fully understood.

Purpose of the Study:

  • To investigate the characteristics and sleep stage-specific distribution of epileptiform discharges in children with Rett syndrome.
  • To explore the potential of sleep electroencephalogram (EEG) features for diagnosing Rett syndrome.

Main Methods:

  • All-night electroencephalograms (EEGs)/polysomnograms were recorded for two consecutive nights in four children (ages 4-11) with Rett syndrome.
  • Analysis focused on the first 10 seconds of each 60-second epoch, quantifying left and right hemisphere spikes.

  • Spike counts were correlated with different sleep stages.
  • Main Results:

    • Spike counts were lowest during wakefulness and significantly higher during non-rapid eye movement (NREM) sleep compared to rapid eye movement (REM) sleep (51-109% increase).
    • Parasagittal regions showed the highest spike frequency across all sleep stages.
    • In three out of four subjects, spikes were most frequent during light NREM sleep (stage 1-2), and their frequency increased in the latter half of the night.

    Conclusions:

    • Epileptiform activity in Rett syndrome is maximally expressed during stage 1-2 NREM sleep and during the early morning hours.
    • Specific sleep EEG features observed during NREM sleep may serve as valuable diagnostic markers for Rett syndrome.
