Statins may trigger autoimmune hepatitis (AIH) in susceptible individuals. This study presents three cases where statin use preceded AIH development, persisting despite drug cessation.

Area of Science:

  • Hepatology
  • Immunology
  • Pharmacology

Background:

  • Autoimmune hepatitis (AIH) etiology is largely unknown, but drug-induced triggers are suspected.
  • Statins have been implicated as potential triggers in isolated case reports.

Observation:

  • Three patients (two male, one female) developed AIH following statin initiation.
  • Clinical presentation included positive antinuclear and antismooth muscle antibodies, and hypergammaglobulinemia.
  • Liver biopsies revealed fibrosis and plasma cell infiltration consistent with AIH.

Findings:

  • Statin-associated AIH was probable in all three cases, meeting International Autoimmune Hepatitis Panel criteria.
  • One patient experienced recurrent AIH upon re-exposure to different statins.
  • AIH persisted in all patients despite statin discontinuation, indicating a potentially severe and ongoing autoimmune process.

Related Experiment Videos

Implications:

  • These findings, along with prior reports, suggest statins can trigger severe, persistent AIH in rare instances.
  • Early recognition and consideration of statin-induced AIH are important for appropriate patient management.
  • Further research into the mechanisms of statin-induced autoimmunity is warranted.
