Area of Science:

  • Neuro-oncology
  • Neurosurgery
  • Neuropathology

Background:

  • Intraventricular tumors can present with non-specific symptoms like chronic headache.
  • Accurate histopathological diagnosis is crucial for appropriate treatment planning.

Observation:

  • A 29-year-old male presented with chronic headache and a left lateral ventricular mass on CT scan.
  • Surgical resection via a transcortical-transventricular approach successfully removed the highly vascular tumor.
  • Initial light microscopy suggested oligodendroglioma, but electron microscopy revealed neuronal features.

Findings:

  • The tumor exhibited characteristics of both oligodendroglioma (clear cytoplasm, perinuclear halos) and central neurocytoma (neuronal elements, vesicles, synaptic structures).
  • This case highlights diagnostic challenges in differentiating intraventricular tumors with overlapping histological features.

Implications:

  • Electron microscopy is essential for definitive diagnosis of ambiguous central nervous system tumors.
  • Accurate diagnosis influences prognosis and therapeutic strategies for intraventricular neoplasms.
