Summary

Distraction osteogenesis successfully lengthened a severely hypoplastic mandible, improving facial aesthetics and function. This innovative treatment corrected significant overjet and enhanced airway space.

  • A case report details the use of a multidirectional extraoral device for mandibular lengthening.
  • Bilateral distraction osteogenesis of the ramus and corpus was performed.
  • A 7-day latency period was followed by gradual distraction and a 12-week consolidation phase.

Findings:

  • Cephalometric analysis showed a decrease in ANB angle from 13 to 6 degrees and overjet from 15 mm to 4 mm.
  • Mandibular corpus length increased from 49 mm to 67 mm, and ramus length from 41 mm to 43 mm.
