Summary

High parathyroid hormone (PTH) levels in dialysis patients do not increase aluminum absorption from the gut. This study suggests PTH may not enhance intestinal aluminum uptake in humans.

Background:

  • Patients with chronic renal failure often have elevated parathyroid hormone (PTH) levels.
  • Aluminum-based phosphate binders are used in hemodialysis patients, raising concerns about aluminum absorption.
  • The relationship between PTH levels and intestinal aluminum absorption is not well understood.

Purpose of the Study:

  • To investigate the effect of high versus low parathyroid hormone (PTH) levels on the intestinal absorption of aluminum.
  • To determine if PTH influences serum aluminum concentrations after oral aluminum hydroxide intake in patients undergoing regular hemodialysis.

Main Methods:

  • Eight patients on regular hemodialysis received a 6-g dose of aluminum hydroxide.
  • Serum aluminum levels were measured before and 5 hours after aluminum hydroxide ingestion.

  • Measurements were taken both before and after parathyroidectomy (PTX) to assess the impact of high and low PTH levels.
  • Main Results:

    • Plasma PTH levels decreased significantly after parathyroidectomy (PTX) (2,724 +/- 1,830 to 352 +/- 63 ng/l).
    • Serum aluminum levels did not show significant changes before or after PTX, nor did they differ significantly between the high and low PTH states.
    • Ingestion of aluminum hydroxide did not lead to a significant increase in serum aluminum levels in relation to PTH status.

    Conclusions:

    • High plasma parathyroid hormone (PTH) levels are not associated with increased serum aluminum concentrations in patients with chronic renal failure.
    • These findings suggest that PTH may not play a significant role in enhancing intestinal aluminum absorption in humans.
    • The study indicates that aluminum hydroxide absorption is not significantly affected by PTH levels in hemodialysis patients.
