Infectious mononucleosis can rarely cause heart complications like myopericarditis. This case highlights a rare instance of myopericarditis developing in an 18-year-old male following Epstein-Barr virus infection.
Area of Science:
- Cardiology
- Infectious Diseases
- Immunology
Background:
- Infectious mononucleosis, primarily caused by Epstein-Barr virus, typically presents with mild symptoms but can lead to rare, severe complications.
- Cardiac complications, including myocarditis and pericarditis, are among the most serious potential sequelae of infectious mononucleosis.
Observation:
- An 18-year-old male presented with symptoms of infectious mononucleosis.
- During hospitalization, clinical, radiographic, and echocardiographic findings indicated the development of myopericarditis.
- The patient received treatment for myopericarditis, including anti-inflammatory drugs and beta-blockers, and was discharged in good condition.
Findings:
- Myopericarditis, an inflammation of the heart muscle and outer lining, is an uncommon complication of acute Epstein-Barr virus infection.
- While transvenous endomyocardial biopsy is the gold standard for diagnosis, it was not performed in this case due to technical reasons.
- The diagnosis of myopericarditis was strongly suspected based on clinical presentation and diagnostic imaging.
Implications:
- Myocarditis associated with Epstein-Barr virus infection may arise from autoimmune mechanisms, potentially involving autoantibodies and complement activation.
- This case underscores the importance of considering cardiac complications in patients with infectious mononucleosis, even when rare.
- Further research into the autoimmune pathways of Epstein-Barr virus-induced myocarditis could inform diagnostic and therapeutic strategies.
