Summary
Soft x-ray radiotherapy offers a promising, long-lasting treatment for severe nail psoriasis. This approach effectively normalized nail plates in a patient with significant thickening, providing relief from functional and cosmetic issues.

- A 51-year-old male patient with severe nail psoriasis and nail plate thickening (up to 30x normal) was treated.
- The patient received fractionated soft x-ray radiotherapy (total 13.5 Gy).
Findings:
- Complete normalization of nail plates was observed 12 months post-therapy.
- The patient remained relapse-free for nearly three years without radiotherapy side effects.
Implications:
- Soft x-ray radiotherapy demonstrates high efficacy and long-lasting remission for severe nail psoriasis with nail thickening.
- This treatment modality warrants consideration for alleviating the functional and psychical burdens of severe psoriatic nail disease.
