A rare primary cardiac osteosarcoma was diagnosed in a 68-year-old woman with pulmonary obstruction. Palliative surgery offered symptom relief for this advanced-stage cancer.
Area of Science:
- Cardiology
- Oncology
- Radiology
Background:
- Osteosarcoma is a rare bone cancer, with primary cardiac involvement being exceptionally uncommon.
- This case highlights the diagnostic challenges and management of a primary cardiac tumor presenting with obstructive symptoms.
Observation:
- A 68-year-old female presented with dyspnea and edema, initially suspected as pulmonary embolism.
- Chest CT revealed a large pulmonary artery mass with metastatic nodules, unresponsive to anticoagulation.
- The patient had severe right ventricular failure, liver congestion, renal dysfunction, and coagulopathy.
Findings:
- Histopathology confirmed primary right ventricle osteosarcoma with pulmonary metastases.
- Palliative tumor resection was performed despite the patient's critical condition.
- Postoperative imaging confirmed the diagnosis and extent of disease.
Implications:
- Primary cardiac osteosarcoma has a poor prognosis, particularly at advanced stages with metastasis.
- Surgical palliation can significantly alleviate obstructive symptoms in patients with advanced cardiac tumors.
- Early recognition and multidisciplinary management are crucial for improving outcomes in rare cardiac neoplasms.
