Summary

Subantimicrobial dose doxycycline (SDD) did not alter the subgingival flora in postmenopausal women with periodontitis over 24 months. No significant increase in doxycycline resistance was observed in the SDD group compared to placebo.

Background:

  • Concerns regarding antimicrobial resistance prompted an evaluation of subantimicrobial dose doxycycline (SDD) safety.
  • The study focused on postmenopausal osteopenic women with periodontitis.

Purpose of the Study:

  • To assess the safety and microbiologic impact of a 24-month regimen of SDD (20 mg twice daily) on subgingival flora.
  • To compare microbial changes in SDD-treated patients versus placebo.

Main Methods:

  • Collected subgingival samples from 128 subjects at baseline and after 24 months.
  • Enumerated bacteria on selective and non-selective media, including doxycycline-containing medium.
  • Identified bacterial species and determined antibiotic susceptibilities.

Main Results:

  • No significant differences in total anaerobic counts or presence of periodontal/opportunistic pathogens between SDD and placebo groups.
  • While total colony-forming units on doxycycline plates increased in the SDD group, the percentage of clinically resistant bacteria did not significantly differ.
  • No significant differences in cross-resistance to doxycycline or other antibiotics were observed between groups.

Conclusions:

  • Twenty-four months of SDD treatment showed no antimicrobial effect on the subgingival flora compared to baseline or placebo.
  • Increased initial resistance to doxycycline did not lead to a significant rise in the percentage of resistant bacteria in the SDD group.
