Observation:
- A 5-year-old female presented with a 3-year history of intermittent left eye (L/E) squinting.
- The squinting episodes occurred daily, starting in the morning and resolving by the next day, with a gradually shortening interval between episodes.
- On examination during the esotropic phase, she exhibited a 50 prism diopter left esotropia with alternate suppression and normal ocular motility.
Findings:
- The patient had normal visual acuity (6/6, N5) and was hypermetropic (+0.50 Dsph) on cycloplegic refraction.
- During non-esotropic periods, she demonstrated BSV with positive stereopsis (40 seconds of arc on Titmus test).
- No abnormalities were found in the anterior or posterior segments of the eyes.
Implications:
- Surgical correction, such as bilateral medial rectus recession or a combination of medial rectus recession and lateral rectus resection, is a viable treatment option.
- Tailoring surgical intervention based on the measured esotropia during symptomatic periods is recommended.
- This case highlights the importance of recognizing the cyclical nature of esotropia for timely and effective management.
