Summary

Background:

  • Laparoscopic partial nephrectomy (LPN) requires precise renal cooling to preserve function.
  • Effective intraoperative cooling methods are critical for minimizing ischemia-reperfusion injury during LPN.

Purpose of the Study:

  • To compare the efficacy of different cooling techniques in porcine kidneys during simulated LPN.
  • To determine the optimal method for maintaining renal parenchymal hypothermia during LPN.

Main Methods:

  • Three cooling strategies were evaluated in 21 porcine kidneys under general anesthesia: ice slush alone, localized 4°C saline infusion, and continuous 4°C saline ureteral irrigation.
  • Renal parenchymal temperature was monitored throughout the procedures following vascular clamping.

Main Results:

  • Continuous ureteral irrigation (Group III) achieved the lowest temperature (10.6°C) and maintained hypothermia for the longest duration (79 minutes).
  • Ice slush (Group I) and localized saline infusion (Group II) showed similar, less effective cooling profiles, with temperatures exceeding 20°C significantly earlier.

Conclusions:

  • Combined ice slush and continuous ureteral irrigation represent an effective cooling strategy for LPN, particularly in complex scenarios.
  • Additional cooling with infused water may be beneficial when the renal collecting system is intentionally opened during surgery.
